At the end of each experiment the strips were measured and weighted, then fixed in 8 0 neutral formaldehyde, embedded in paraffin, and examined histologically . Thus the muscular nature of the arterial strips was checked and in the parenchymal strips the number of vessels and bronchi measuring 100 m or more in diameter was counted . RESULTS.

However, the knowledge of additional factors associated with the prognosis might be helpful in planning the therapy for an individual patient. Methods : We investigated paraffin-embedded specimens of 25 patients undergoing surgical treatment for adenocarcinoma by immunohistochemistry to assess the expression and the prognostic impact of p53, Ki-67, Bcl-2, Fas and MMP-9. The number of positive neoplastic cells and the intensity of staining were scored semiquantitatively. The presence of micrometastases in locoregional lymph nodes was studied by staining for low molecular weightcytokeratins. Neither survival time nor disease-free survival time were known at the time of the investigation. Results : No correlation was established between immunohistochemically detected micrometastases and prognosis.The expression of p53, Ki-67, Bcl-2 and MMP-9 did not influence the survival or disease-free survival. Expression of Fas, however, was associated with longer disease-free survival P 0.014 ; . Conclusion : Fas exerts an influence on prognosis in adenocarcinoma of the pancreas. However, further investigations in larger patient samples are required to confirm these results.

INTRODUCTION A new Swedish book of more than 800 pages in two volumes is an ambitious attempt to evaluate by meta-analytical techniques treatments of alcohol and other addictive substances SBU, 2001 ; . This is the main focus, although the report also reviews brief interventions to reduce alcohol intake, long-term prognosis of substance dependence, obstetric questions and economic aspects of addiction treatments. This book is the focus of this Commentary. A team of 11 experts, seven reviewers and two co-ordinators has produced this twin book. The employer is the Swedish Council on Technology Assessment in Health Care abbreviated to `SBU' in Swedish ; . Their yellow book series includes not only the assessment made by the expert group but, based on the former, the Council's executive conclusions and recommendations for action in practice. Thus, this magnum opus might have great influence on treatment practice in future and pbz.

Cussed with the science community before crossing over into the mainstream media following a Mayo Clinic report in 2000. In Waddle v. Galen of Kentucky, Inc., 346 the court of appeals addressed the issue of vicarious liability of the principal-hospital when the plaintiff releases the agent-doctor. 347 Plaintiff, Gerold Waddle, was injured in a motorcycle accident and was treated by Dr. Brown at a hospital owned by Galen of Kentucky. 348 Plaintiff alleged that Dr. Brown negligently delayed his surgery, causing complications. 349 Plaintiff brought a medical malpractice action against Doctor Brown and Galen. 350 Plaintiff's claim did not allege any theory of liability against Galen other than vicarious liability under the theory of ostensible agency. 351 Plaintiff voluntarily dismissed the action against Dr. Brown, 352 apparently after receiving , 900 from Dr. Brown's insurance carrier as compensation for his costs. 353 Plaintiff intended to proceed solely against Galen. 354 However, the trial court dismissed the claim, holding that the voluntary dismissal of Dr. Brown released Galen of its liability as principal under the theory of ostensible agency. 355 The plaintiff appealed arguing that the voluntary dismissal was not a release, and, alternatively, that a release of the agent does not necessarily operate as a release of the principal. 356 In rejecting these arguments, a unanimous court first held that the oral agreement to dismiss the claim against Dr. Brown in exchange for , 900 was a release because it was a binding contract supported by valuable consideration. 357 Next, the court relied on Copeland v. Humana of Kentucky, Inc. 358 in holding that the release precluded plaintiff from recovering against Galen on the theory of vicarious liability. 359 Since Galen's liability as principal was derived solely from the alleged negligence of its agent, Dr. Brown, the release of Dr. Brown barred recovery from Galen on the theory of vicarious liability. 360 The court found that plaintiff's reliance on Cohen v. Therefore, the present study has been restricted to cycles in which fresh embryos selected for replacement or cryopreservation had similar characteristics before cryopreservation, thus allowing for meaningful comparisons. Our data could be explained by maternal inuences, embryorelated differences or a combination of both factors. With regard to maternal inuences, it has been demonstrated that women who had a previous pregnancy have a signicantly higher live birth rate after IVF treatment than women with no previous pregnancies, even after adjusting for age Templeton et al., 1996 ; . Likewise, women who achieved a previous IVF pregnancy were also found to have a higher chance of pregnancy on subsequent attempts compared with those not conceiving Tan et al., 1994; Molloy et al., 1995; Croucher et al., 1998; Templeton and Morris, 1998 ; . The present study extends these observations and conrms that women who have achieved a pregnancy in a fresh IVF ICSI cycle have a superior chance of conception in the related frozenthawed cycle. These women are likely to be more fertile and possess more efcient reproductive mechanisms that facilitate implantation as evidenced by the occurrence of conception in their fresh attempt. This postulate is supported by the study of Simon et al. 1993 ; which suggested that patients who conceived in an IVF cycle have an enhanced endometrial receptivity that is resistant to the negative effects of superovulation, offering them an improved prognosis in subsequent attempts, compared with patients who conceived naturally but were subsequently infertile. The present study results could also be explained on the basis of differences in embryo quality. There is considerable evidence that embryo morphology prior to cryopreservation impacts on the outcome of the cryo-thawed cycle Mandelbaum et al., 1988; Schalkoff et al., 1993; Kondo et al., 1996; Karlstrom et al., 1997; Lightman et al., 1997 ; . In this study, fresh embryos were selected according to dened morphological criteria Staessen et al., 1992; Steer et al., 1992; Gerris et al., 1999; Van Royen et al., 1999 embryos that have reached 4 cells by day 2 or 68 cells by day 3 of in-vitro culture and showed symmetrical blastomeres with single nuclei and `10% fragmentation were considered of high quality and selected for either transfer or cryopreservation. This ensured a fairly similar quality among the embryo population studied. The study groups were also comparable with respect to postthaw embryo survival and the proportion of thawed cycles in which all transferred embryos survived fully intact and showed no blastomere loss Edgar et al., 2000 ; . Nevertheless, cryothawed embryos derived from successful fresh cycles were more able to divide after thawing, leading to a signicantly higher proportion of cycles in which only embryos that had divided after thawing were transferred P 0.04 ; .
