A vaccine effective against all 4 serotypes of dengue virus is urgently needed. Several approaches for a tetravalent dengue vaccine are being studied; the most advanced being the live-attenuated viruses LAV ; which have been tested in tetravalent formulations in human subjects. Phase 1 and 2 studies of tetravalent LAV vaccines are ongoing and will be discussed. Other approaches are in preclinical development.These include other liveattenuated viruses, inactivated viruses, DNA vaccines, recombinant envelop proteins and adenovirus vectored vaccines. There are significant obstacles to dengue vaccine development. First, there is no animal model for disease. Second, the current in vitro assay for of protective immunity, neutralizing antibody, is problematic.Thirdly, interference among the 4 serotypes of viruses may cause sub-optimal immunogenicity of the tetravalent vaccine.The most significant obstacle to vaccine development is poor understanding of the pathogenesis of DHF, wherein prior exposure to one serotype of dengue virus predisposes the individual to DHF upon exposure to a heterotypic serotype.This phenomenon suggests dengue vaccine safety should be monitored for prolonged period of time in the population. Further basic research is urgently needed in this area.
